Complete the form below to determine your eligibility for financial assistance to utilize our services. Decisions will be made upon review of your completed application. Please note that not all applications will be awarded financial aid.

Instructions:

Please provide the following required materials for your application to be considered for review:

• Completed online financial aid application form

• An official letter from your institution stating you are eligible for free or reduced lunch (high school student) and/or qualify for and receive federal financial aid (college student)

• A personal statement of 150 words maximum describing your interest in pursuing a career in STEM and/or medicine and how a financial aid award to supplement the cost to utilize our services will help you achieve your goal
